Please note that if insurance information is NOT correct you will be entered into the database as self pay. Once corrected information can be obtained we will fix it for you. Asking your therapist to fill out FMLA forms or other employee or government paperwork my result in additional cost, set by the provider. There is no charge for enrollment.
