This process includes:
- Completing the information requested during registration and once logged in,
- Navigating to the DOCUMENTS tab and completing all forms found under the item REQUIRED FORMS.
- Once the required forms have been submitted navigate to the APPOINTMENT tab and request an appointment at one of the available times.
- Email or text us the following information:
- A copy of the front and back of your State or Military Issued ID.
- A copy of the front and back of your Insurance cards.
- Once we receive your ID and Insurance Cards, staff will verify your coverage and call you to schedule confirm the appointment time.
